Checklist

OCC in Review: Required Components Checklist

1. Unrestricted Medical License

    • Each active license is unrestricted

2. Lifelong Learning and Self-Assessment

    • CME requirements met for current 3 year cycle
    • eJournal submissions are completed
    • POMT modules are completed
    • Other method documentation submitted to board (if appropriate)

3. Demonstration of Cognitive Expertise

    • Osteopathic Continuous Certification exam passed

4. Practice Performance Assessment

    • CAP programs completed for the current cycle
    • Communications Module completed
    • Practice Performance Self-Appraisal documents submitted (if appropriate)
    • Teaching/Research/Administrative Practice Self-Assessment documents submitted (if appropriate)
    • Attestation forms submitted for ABMS MOC activity approval (if appropriate)

5. Professional Association

    • AOA membership current
